General Factors


   Cigarette smoking (especially more than 10 years of one pack of cigarettes per day)
   Chronic bronchitis
   Older age plus concomitant illness (not age alone)
   Alcohol abuse
   Abnormal chest x-ray
Surgical factors
   Thoracic surgery (especially with resection of functional lung)
   Upper abdominal surgery
   Anesthesia time exceeding 3 to 4 hours, anesthesia type
   Intraoperative pancuronium
Pulmonary function (variable predictive value)
   Forced vital capacity <70% of predicted
   Forced expiratory volume in 1 second <70%
of predicted
   Forced expiratory volume in 1 second/forced vital
capacity <65%
   Maximum voluntary ventilation <50% of predicted
   Diffusing capacity of carbon monoxide <50%
of predicted
V·O2max <15 mL/min/kg
Arterial blood gases
Paco2   >45 mm Hg
   Hypoxemia not a reliable indicator
Hemoglobin level >16 g/dL
Asthma
Myasthenia gravis
Spinal cord injury
Acute respiratory infections
