Usually, the patient with CGD displays signs and symptoms associated with infections of the skin, lymph nodes, lung, liver, and bone by age 2. Skin infection is characterized by small, well-localized areas of tenderness. Seborrheic dermatitis of the scalp and axilla is also common. Lymph node infection typically causes marked lymphadenopathy with draining lymph nodes and hepatosplenomegaly. Many patients develop liver abscess, which may be recurrent and multiple; abdominal tenderness, fever, anorexia, and nausea point to abscess formation. Other common infections include osteomyelitis, which causes localized pain and fever, pneumonia, and gingivitis with severe periodontal disease.

Warm phase: pink, flushed skin; low urine output; blood pressure normal or slightly elevated; rapid, bounding pulse; rapid, shallow respirations; altered level of consciousness (anxiety, irritability, shortened attention span, agitation). Cool phase: pale, cool, mottled skin; decreased level of consciousness; obtundation; rapid, shallow respirations; peripheral pulses rapid, weak and thready, possibly irregular, or absent; low blood pressure

- General goal: Decide whether the patient’s type of infections (osteomyelitis, perirectal abscess) and infecting organisms are consistent with the diagnosis of CGD.
- Order a DCF or DHR assay.
- If abnormal, initiate sulfamethoxazole/trimethoprim prophylaxis.

Article Excerpts About Symptoms of Chronic Granulomatous Disease:
Doctors may suspect CGD in
male infants between three months and two years of age who have had
fevers, skin rashes, persistent cough, boils, gum disease, swollen
glands or lymph nodes, and enlarged liver and spleen. However,
patients with CGD may not develop symptoms until as late as
adolescence. Repeated infections can cause tumor-like masses or
“granumlomas” to develop in the skin, lungs, lymph nodes, liver, or
bones. Granulomas can cause blockage of the gastrointestinal or
urinary tracts. The lesions tend to heal slowly and to drain for a
