IV. Clinical Presentation

The clinical presentation of patients with methemoglobinemia depends on the
serum concentrations of both hemoglobin and methemoglobin. Increasing
methemoglobin levels are associated with progressively worsening symptoms.
Patients with lower serum hemoglobin concentrations are affected at lower
levels of methemoglobin. Patients with methemoglobin concentrations lower than
10% rarely have symptoms unless they are already anemic. Most patients with
concentrations of methemoglobin between 10% and 25% have cyanosis but few other
symptoms. Depending on the degree of cyanosis, metabolic acidosis may also
develop. Levels from 30% to 50% are associated with confusion, dizziness,
fatigue, headache, tachypnea, and tachycardia. Levels greater than 50% are
associated with severe acidosis, arrhythmias, seizures, lethargy, and coma.
Lethal levels occur at methemoglobin concentrations of about 70%.

Signs and symptoms of SVT depend on the age at presentation and the duration of
the tachycardia. Episodes of SVT may last only a few seconds or may persist for
hours. Many children tolerate these episodes extremely well, and it is unlikely
that short paroxysms are dangerous. Infants with SVT often present with heart
failure after the tachycardia goes unrecognized for a prolonged period.
Episodes lasting longer than 6 to 24 hours may result in an acutely ill child
with evidence of cardiopulmonary distress, with tachypnea, poor feeding,
vomiting, lethargy, ashen color, restlessness, and irritability. Physical
findings in such cases include pallor, tachypnea, diaphoresis, and
hepatomegaly.
Older children may complain of lightheadedness, chest tightness, palpitations,
and fatigue. Chest pain or discomfort is less common. The patient may become
faint, dizzy, or even syncopal. If the HR is exceptionally rapid or if the
attack is prolonged, heart failure may ensue.

The clinical presentation of patients with Hirschsprung's disease depends on the length of the aganglionic segment. Ninety-four percent
of all patients with Hirschsprung
's disease fail to pass meconium within the first 24 hours of life. Other
symptoms include constipation (93%), vomiting (64%), diarrhea (25%), rectal
fissures with bleeding (5%), and bowel perforation (3%). Abdominal distention
occurs in 83%, and rectal examination reveals an empty rectal vault in 60%
(especially with long-segment Hirschsprung
's disease) or rectal impaction in 7% (especially with short-segment Hirschsprung's disease).

Cyclic Vomiting Syndrome: NIDDK (Excerpt)
CVS has four phases:
- Prodrome
- Episode
- Recovery
- Symptom-free interval
The prodrome phase signals that an episode of nausea and
vomiting is about to begin. This phase, which is often marked by abdominal
pain, can last from just a few minutes to several hours. Sometimes taking
medicine early in the prodrome phase can stop an episode in progress.
However, sometimes there is no warning: A person may simply wake up in the
morning and begin vomiting.
The episode phase consists of nausea and vomiting; inability to
eat, drink, or take medicines without vomiting; paleness; drowsiness; and
exhaustion.
The recovery phase begins when the nausea and vomiting stop.
Healthy color, appetite, and energy return.
The symptom-free interval phase is the period between episodes
when no symptoms are present.

Cyclic Vomiting Syndrome: NIDDK (Excerpt)
The main symptoms of CVS are severe vomiting, nausea, and retching
(gagging). Episodes usually begin at night or first thing in the morning
and may include vomiting or retching up to five or six times an hour
during the worst of the episode. Episodes usually last anywhere from 1 to
4 days, though they can last for up to 10 days.
Other symptoms include pallor, exhaustion, and listlessness. Sometimes
the nausea and vomiting are so severe that a person appears to be almost
unconscious. Sensitivity to light, headache, fever, dizziness, diarrhea,
and abdominal pain may also accompany an episode.
In addition, the vomiting may cause drooling and excessive thirst.
Drinking water usually leads to more vomiting, though the water can dilute
the acid in the vomit, making the episode a little less painful.
Continuous vomiting can lead to dehydration, which means that the body has
lost excessive water and salts.
