In dilated cardiomyopathy, the heart ejects blood less efficiently than normal. Consequently, a large volume of blood remains in the left ventricle after systole, causing signs of heart failure — both left-sided (shortness of breath, orthopnea, dyspnea on exertion, paroxysmal nocturnal dyspnea, fatigue, and an irritating dry cough at night) and right-sided (edema, liver engorgement, and jugular vein distention). Dilated cardiomyopathy also produces peripheral cyanosis and sinus tachycardia or atrial fibrillation at rest in some patients secondary to low cardiac output. Auscultation reveals diffuse apical impulses, pansystolic murmur (mitral and tricuspid insufficiency secondary to cardiomegaly and weak papillary muscles), and S 3 and S4 gallop rhythms.

Clinical features of the disorder may not appear until it’s well advanced, when atrial dilation and, possibly, atrial fibrillation abruptly reduce blood flow to the left ventricle. Reduced inflow and subsequent low output may produce angina pectoris, arrhythmias, dyspnea, orthopnea, syncope, heart failure, and death. Auscultation reveals a medium-pitched systolic ejection murmur along the left sternal border and at the apex; palpation reveals a peripheral pulse with a characteristic double impulse (pulsus biferiens) and, with atrial fibrillation, an irregular pulse.

Because it lowers cardiac output and leads to heart failure, restrictive cardiomyopathy produces fatigue, dyspnea, orthopnea, chest pain, generalized edema, liver engorgement, peripheral cyanosis, pallor, S3 or S4 gallop rhythms, and systolic murmurs of mitral and tricuspid insufficiency.

Cardiomyopathy, hypertrophic:

The primary signs and symptoms of hypertrophic cardiomyopathy are exertional dyspnea (90% of patients), angina (75% of patients), and syncope. The chest pain is atypical in that it usually occurs at rest, is prolonged, has no relation to exertion, and isn’t relieved by a nitrate. When sudden death occurs, it may be the first indication of the disease.
Signs and symptoms of the disorder may not appear until it’s well advanced, when atrial dilation and, possibly, atrial fibrillation abruptly reduce blood flow to the left ventricle. Reduced inflow and subsequent low output may produce angina pectoris, arrhythmias, dyspnea, orthopnea, syncope, heart failure, and death.
Clinical tip The most common symptoms of hypertrophic cardiomyopathy are dyspnea and chest pain. Syncope is also common, usually occurring after exertion.
Auscultation reveals a medium-pitched systolic ejection murmur along the left sternal border and at the apex. This murmur increases with Valsalva’s maneuver and decreases with squatting. Palpation reveals a peripheral pulse with a characteristic double impulse (pulsus biferiens) and, with atrial fibrillation, an irregular pulse and loud S4.

In the early stages of all 3 forms of cardiomyopathy, the symptoms are nonspecific and can mimic other disease processes. The cardiac examination can be completely normal. Therefore, those patients who raise suspicion for this disease either by family history or clinical presentation should be carefully evaluated.

Dilated cardiomyopathy can be present for several years without causing
significant symptoms. With time, however, the enlarged heart gradually weakens.
This condition is commonly called "heart
failure ," and it is the hallmark of dilated cardiomyopathy. Typical signs
and symptoms of heart failure include: fatigue; weakness; shortness of breath,
sometimes severe and accompanied by a cough, particularly with exertion or when
lying down; and swelling of the legs and feet, resulting from fluid accumulation
that may also affect the lungs
(congestion) and other parts of the body. It also produces abnormal weight
gain. (The cough and congestion mimic and, therefore, can be misdiagnosed as
pneumonia or acute bronchitis. Also, heart
failure is often from heart disease other than cardiomyopathy.)
Because of the congestion, some physicians use the older term "congestive
cardiomyopathy" to refer to dilated cardiomyopathy. In advanced stages of the
disease, the congestion may cause pain in the chest or abdomen.
In advanced stages, some patients develop irregular heartbeats, which can be
