TESTICULAR PAIN: Excerpt from Algorithmic Diagnosis of Symptoms and Signs

It is rare for pain in the testicle to occur without a mass. Therefore, the algorithmic diagnosis is essentially the same as that of testicular swelling. Keep in mind that there are cases of orchitis, epididymitis, and torsion not associated with testicular swelling. Also, in cases of renal colic (nephrolithiasis, etc.) and herniated lumbar disk, pain may be referred to the testicle.
