Primary Hypothyroidism


Addison's disease (reversible hypothyroidism)
Addition of other drugs to levothyroxine therapy
   Aluminum hydroxide–containing antacids
   Bile acid sequestrants
   Ferrous sulfate
   Sertraline
   Sucralfate
Destruction of the thyroid gland
   External beam radiation therapy to the head/neck
   Radioactive iodine (may be transient)
   Replacement of thyroid tissue
       Infiltrative diseases
          Amyloidosis
          Chondrocalcinosis
          Cystinosis
          Hemochromatosis
          Histiocytosis X
          Riedel's thyroiditis
          Sarcoidosis and other granulomatous diseases
          Scleroderma
       Malignancies
          Lymphoma of the thyroid
          Metastatic nonthyroidal cancer
          Thyroid cancer
   Surgery (total or subtotal thyroidectomy) (may be transient)
   Thyroiditis
   Acute (suppurative) thyroiditis [bacterial, mycobacterial, fungal, parasitic, gummatous, Pneumocystis carinii   infection in acquired immunodeficiency syndrome (AIDS)]
       Chronic
          Atrophic thyroiditis
          Hashimoto's thyroiditis
       Due to drug therapy
          Amiodarone
       Interferon-α

          Interleukin-2
          Lymphokine-activated killer cell therapy
       Transient
          Postpartum thyroiditis
          Silent (painless) thyroiditis
          Subacute (DeQuervain's, granulomatous) thyroiditis
Generalized resistance to thyroid hormone
Inhibition of thyroid hormone synthesis or accelerated disappearance from the circulation
   Exposure to exogenous goitrogens
       
   Inherited defects of thyroid hormone synthesis
   Iodide deficiency
   Selenium deficiency (usually with concomitant iodide
deficiency)
   Thyroid agenesis, dysgenesis, or ectopy
Thyroid growth-blocking antibodies
Transplacental passage of antithyroid drugs, chemicals, or agents
TSH hyporesponsiveness
Abnormal G-protein (Gs   activity)
       Pseudohypoparathyroidism type Ia
   TSH receptor abnormalities
   TSH receptor-blocking antibodies

Secondary (Pituitary) or Tertiary (Hypothalamic) Hypothyroidism*


   After withdrawal of thyroid hormone therapy in euthyroid patients (transient)
   Thyroid-releasing hormone deficiency or insensitivity
   TSH synthetic defect
