Granulocytopenia and lymphocytopenia:
Signs and symptoms
(Professional Guide to Diseases (Eighth Edition))
Characteristically, patients with granulocytopenia experience slowly progressive fatigue and weakness; however, if they develop an infection, they can exhibit sudden onset of fever and chills and mental status changes. Overt signs of infection (pus formation) are usually absent. Localized infection can quickly become systemic (bacteremic) or spread throughout an organ (pneumonia). All patients should be meticulously evaluated for even subtle signs of infection because untreated infections can lead to septic shock in 8 to 24 hours. If granulocytopenia results from an idiosyncratic drug reaction, signs of infection develop abruptly, without slowly progressive fatigue and weakness.
Patients with lymphocytopenia may exhibit enlarged lymph nodes, spleen, and tonsils and signs of an associated disease.

Patients with granulocytopenia typically experience slowly progressive fatigue and weakness, followed by the sudden onset of signs of overwhelming infection (fever, chills, tachycardia, anxiety, headache, and extreme prostration); ulcers in the mouth or colon; pharyngeal ulceration, possibly with associated necrosis; pneumonia; and septicemia, possibly leading to septic shock and death.
If granulocytopenia is caused by an idiosyncratic drug reaction, signs of infection develop abruptly, without slowly progressive fatigue and weakness.
Patients with lymphocytopenia may exhibit enlarged lymph nodes, spleen, and tonsils and signs of an associated disease.

- Fever (temperature should not be taken rectally) tachycardia, and hypotension may indicate systemic infection.
- Oral ulceration, gingival irritation, pharyngitis, thrush
- Cellulitis, perirectal, or labial abscesses
- Hepatomegaly or splenomegaly
- Bruises, petechiae, pallor (other cell lines may be involved)
- Phenotypic abnormalities (thumb anomalies, dwarfism, joint findings)
- Systemic infection: Fever, rash, upper respiratory symptoms, jaundice
