The disease occurs worldwide, but is more common in Europe, Australia, and New Zealand, where it’s seen in up to 5% of the elderly population. Although its exact cause is unknown, one theory holds that early viral infection causes a dormant skeletal infection that erupts many years later as Paget’s disease. Genetic factors are also suspected.

The term 'incidence' of Paget's disease of bone refers to the annual diagnosis rate, or the number of new cases of Paget's disease of bone diagnosed each year. Hence, these two statistics types can differ: a short-lived disease like flu can have high annual incidence but low prevalence, but a life-long disease like diabetes has a low annual incidence but high prevalence.
