Common Misdiagnoses and Surgical errors/complications

No biopsy to be done in juvenile angiofibroma: Juvenile angiofibroma occurs in adolescent males. Exact aetiology of the condition is not known although it is thought to be due to testosterone. The patient presents with epistaxis, nasal obstruction, rhinorrhea and sometimes anosmia conductive deafness. It has to be distinguished from other conditions such as a pyogenic granuloma, nasal polyp, cysts and carcinoma. A history of allergy can be asked for in cases of nasal polyps. This can be done with the help of investigations such as CT scan. Biopsy is contraindicated in cases of juvenile polyp as it can lead to severe haemorrhage.

Aortic stenosis in the elderly: Aortic stenosis is a condition characterised by the calcification of a normal valve and is more common in the elderly population. This is the most common cause.the patient usually presents with angina, syncope and dyspnea from congestive heart failure. The condition can be misdiagnosed as a case of aortic valve sclerossis, hypertrophic obstructive cardiomyopathy, mitral regurgitation and pulmonary stenosis. Chest ray, ECG and echocadiography help in diagnosis. Patients must receve infective endocarditis. surgery is advised when there is gross reduction of the valve orifice.

Mitral regurgitation due to rheumatic fever: Mitral regurgitation occurs most commonly in men due to rheumatic fever and dilattaion of the left ventricle. It is characterised by left ventricular failure features such as dyspnea, orthopnea and paroxysmal nocturnal dyspnea. With severe and chronic mitarl regurgitation right- sided failure occurs and pulamonary hypertension maybe a late finding. It must be differentiated from ventricular septal defect, hypertrophic obstructive cardiomyopathy and aortic stenosis. Medical treatment is aimed at relieving symptoms by increasing the cardiac output. For significantly limiting symptoms surgery must be considered.

Cystic hydatid disease a parasitic infestation: Cystic hydatid disease is a parasitic infestation caused by the larval/ cyst stage of the Tapeworm ( Echinococcus granulosus) . Dog is the definitive host of this parasite, sheep are the usual intermediate host but humans are accidental intermediate hosts. The organs commonly infested by this parasite are the liver and the lung. Usually the cysts are single and involve the right lobe of the liver. It should be differentiated from other hepatic cysts, liver abcess, biliary obstruction, lung abcess, etc. Ultrasound and CT scan can diagnose the cystic lesion, serodiagnostic assay (ELISA) is helpful in 85% of patients. The treatment consists of PAIR (percutaneous aspiration, infusion of scolicidal agents and reaspiration) for anatomically and surgically appropriate lesions. Surgery remains the treatment of choice for cysts where PAIR is not possible.
