Bleeding severity can range from mild local bleeding at a small injury to massive hemorrhage. Internal bleeding may also show via blood in stool, blood in urine, or gastrointestinal bleeding. Other common sites for bleeding symptoms include nosebleeds, digestive bleeding, vaginal bleeding (and dysmenorrhea), rectal bleeding, and bleeding gums. Bruising is also related to bleeding. Excessive unexplained bleeding or bleeding easily can be caused by numerous diseases including bleeding disorders and several severe diseases (e.g. Leukemia). Any type of bleeding is a severe symptom that needs prompt professional medical diagnosis.

  • Physical examination
    • Examine for petechial body rash (pinhead size red rash) which is common in any disorder that might cause thrombocytopenia (reduced platelets), platelet dysfunction or vasculitis
    • Examine for bruises which may suggest Hemophilia, Christmas disease, platelet disorders or disseminated intravascular coagulation
    • Examine for bleeding gums - if bleeding gums is present with skin bruises suggests platelet disorders or disseminated intravascular coagulation.
    • Look for gum hypertrophy (enlargement) which occurs in monocytic Leukemia, pregnancy, scurvy, gingivitis and in patients taking phenytoin

  • Is bleeding from multiple sites? - suggests the presence of a systemic bleeding defect.
  • Is bleeding spontaneous? - suggests the presence of a systemic bleeding defect.
  • Does bleeding occur immediately after trauma or is it delayed? - if immediate suggests platelet defect; if delayed i.e. 24 hrs after trauma it suggests a coagulation factor deficiency.
