Diagnostic tests for Bulging eyes:

Various tests are used in the diagnosis of Bulging eyes. Some of these are listed below :

  • Physical examination
    • Fever may suggest cavernous sinus thrombosis
    • Examine the eyes for protrusion of the eyeball out of the orbit. This may be very obvious but if not, look carefully at the sclera (white part of the eye) which will not be covered by the lower eyelid. Also look from behind over the patient's forehead where the eye may be visible in front of the eyebrow ridge
    • Examine for complications of bulging eyes including conjunctivitis (eye infection), corneal ulceration (due to inability to close eyelids), optic nerve atrophy or paralysis of one or more of the muscles that move the eye
    • Examine for other signs of Hyperthyroidism e.g. fine tremor, increased pulse rate, thyroid gland enlargement, heart murmur - may indicate Grave's disease is cause of bulging eyes.

During a consultation, your doctor will use various techniques in their assessment of Bulging eyes. Your doctor may ask several questions when assessing your condition. It is important to remember that your consultation is a two-way process and any extra information you can share with your doctor about your symptoms may help them with their diagnosis. Some of the questions your doctor may ask are listed below:

  • How long have you noticed the bulging eyes? - to determine if acute or chronic.
  • Are one or both eyes affected? - If bilateral suggests Graves's disease. If unilateral may suggest tumors of the orbit (e.g. dermoid, optic nerve glioma, neurofibroma, granuloma), Cavernous sinus thrombosis, Grave's disease or pseuotumors of the orbit.
  • Recent medical history? - e.g. sinusitis or ear infections may be complicated and followed by Cavernous sinus thrombosis which may cause unilateral bulging eye.
  • Past medical history? - e.g. risk factors for Cavernous sinus thrombosis include congenital heart disease, systemic cancer, polycythaemia and sickle cell anemia.
  • Medications? - oral contraceptive pill may increase risk of Cavernous sinus thrombosis.
