Diagnostic tests for Dry eye:

Various tests are used in the diagnosis of Dry eye. Some of these are listed below :

  • Physical examination
    • Inspect the eyes and look for the presence of any foreign bodies or eyelid blepharitis
    • Fluorescein and Rose Bengal staining of the surface of the eye may show small defects in the cornea suggesting dry eyes or corneal ulceration
    • Measure visual acuity
    • Central nervous system examination, including cranial nerves looking for signs of 7th nerve palsy (which may be due to brain stem stroke, brain stem tumor or multiple sclerosis) which causes inability of the eyelid to close forcefully on the affected side and consequent dry eye
    • Look for signs of Bell's palsy (7th nerve palsy of unknown cause) e.g. unable to wrinkle forehead or close eye forcefully on affected side, unable to whistle or blow out cheeks

Diagnosis Checklist: Questions your doctor may ask (and why!)

During a consultation, your doctor will use various techniques in their assessment of Dry eye. Your doctor may ask several questions when assessing your condition. It is important to remember that your consultation is a two-way process and any extra information you can share with your doctor about your symptoms may help them with their diagnosis. Some of the questions your doctor may ask are listed below:

  • How long have you had problems with dry eyes? - to establish if acute or chronic.
  • Is it unilateral or bilateral? - Unilateral dry eye may suggest foreign body, facial nerve (7th nerve) palsy, Bell's palsy or Herpes zoster oticus.
  • What is the age of the person with dry eyes? - elderly people often complain of a chronic gritty sensation due to reduced tear secretion with age.
  • Are there simple reasons for the dry eyes? - e.g. wearing contact lenses, excessive reading, excessive computer usage, windy conditions, dry weather conditions, air travel (because the air in planes is dry), air-conditioning (air conditioners remove moisture from the air) and home heating.
  • Is there a chance there may be something in your eye, or do you feel like there is something in your eye? - may suggest presence of foreign body.

Diagnosis Checklist: Associated symptoms your doctor may ask about (and why!)

Sometimes, other symptoms may be present and may help your doctor analyse your condition. The symptoms your doctor may ask about (in association with Dry eye) may include:

  • Symptoms of Primary Sjogren's syndrome? - e.g. dry eyes, dryness of the mouth, skin or vagina. This syndrome may be associated with many systemic conditions such as Raynaud's phenomenon, difficulty in swallowing (as seen in systemic sclerosis), painful joints (like that seen with systemic lupus erythematosus), thyroid disease, myasthenia gravis, primary biliary cirrhosis, chronic active hepatitis, renal diabetes insipidus, renal tubular acidosis and vasculitis.
  • Symptoms of Rheumatoid arthritis? - e.g. joint pain, swelling, deformity and morning stiffness. It may also cause a number of systemic symptoms including carpal tunnel syndrome, anemia and enlarged spleen. 15% of people with Rheumatoid arthritis have a secondary Sjogren's syndrome which comprises dry eyes and dry mouth.
  • Menopausal symptoms? - e.g. palpitations, hot flushes, night sweats, lightheadedness, dry skin, dry vagina, mood change. - may also experience dry eyes which is thought to be due to estrogen withdrawal.
  • Symptoms of Bell's palsy? - e.g. marked weakness on one side of the face, mouth sags, drooling from the affected side of mouth, loss of taste on front of tongue, eye may water, pain behind the ear, incomplete closure of the eye and resultant dry eye.
  • Symptoms of Grave's disease - e.g. preference for cooler weather, increased appetite, heart palpitations, increased sweating, nervousness, irritability, diarrhea, lack of menstrual periods and proptosis (forward displacement of the eyeball) with resultant dry eyes.
