Diagnostic tests for Eye blinking symptoms:

Various tests are used in the diagnosis of Eye blinking symptoms. Some of these are listed below :

  • Physical examination
    • Inspect the eyes and look for the presence of any foreign bodies
    • Fluorescein and Rose Bengal staining of the surface of the eye may show small defects in the cornea suggesting dry eyes or corneal ulceration
    • Measure visual acuity
    • Central nervous system examination, including cranial nerves looking for signs of 7th nerve palsy (which may be due to brain stem stroke, brain stem tumor or multiple sclerosis) which causes inability of the eyelid to close forcefully on the affected side and consequent dry eye
    • Look for signs of Bell's palsy (7th nerve palsy of unknown cause) e.g. unable to wrinkle forehead or close eye forcefully on affected side, unable to whistle or blow out cheeks
    • If 7th nerve palsy, examine for a blister-like rash on the pharynx or external ear canal that would suggest herpes zoster (shingles) infection of the nerve

Diagnosis Checklist: Questions your doctor may ask (and why!)

During a consultation, your doctor will use various techniques in their assessment of Eye blinking symptoms. Your doctor may ask several questions when assessing your condition. It is important to remember that your consultation is a two-way process and any extra information you can share with your doctor about your symptoms may help them with their diagnosis. Some of the questions your doctor may ask are listed below:

  • How long have you had the blinking symptoms? - to determine if acute or chronic.
  • What is the actual problem with the blinking? - inability to blink the eye, excessive blinking of the eye or reduced frequency of spontaneous blinking of the eye. Inability or difficulty in blinking the eye may be due to Bell's palsy or ptosis. Excessive blinking may be due to Tourette syndrome, tardive dyskinesia, benign essential blepharospasm, anxiety, dry eyes, foreign body in the eye or eye infection. Reduced frequency of spontaneous blinking of the eye is seen in Parkinson's disease.
  • Is it unilateral or bilateral? - Unilateral inability to blink the eye may suggest facial nerve (7th nerve) palsy, Bell's palsy or Herpes zoster oticus. Bilateral inability to blink the eye may suggest proptosis(forward displacement of the eyeball) due to Graves disease.
  • Medications? - e.g. all antipsychotic medications may cause tardive dyskinesia as a serious and potentially irreversible side effect. The older antipsychotic medications appear more likely to produce tardive dyskinesia than the newer atypical antipsychotic. The longer these drugs are taken, the greater the chance of developing the side effect.

Diagnosis Checklist: Associated symptoms your doctor may ask about (and why!)

Sometimes, other symptoms may be present and may help your doctor analyse your condition. The symptoms your doctor may ask about (in association with Eye blinking symptoms) may include:

  • Symptoms of tardive dyskinesia? - e.g. abnormal involuntary movements of the mouth, lips, tongue and eyelids and possibly involving the limbs and trunk. The movements are worse during times of stress and disappear during sleep.
  • Symptoms of Bell's palsy? - e.g. marked weakness on one side of the face, mouth sags, drooling from the affected side of mouth, loss of taste on front of tongue, eye may water, pain behind the ear, incomplete closure of the eye and resultant dry eye.
  • Symptoms of Graves disease - e.g. preference for cooler weather, increased appetite, heart palpitations, increased sweating, nervousness, irritability, diarrhea, lack of menstrual periods and proptosis (forward displacement of the eyeball) with resultant incomplete ability to blink eye and dry eyes.
  • Symptoms of Parkinson's disease? - e.g. tremor of the fingers and thumbs, slowness of movement, stiff aching limbs, difficulty with fine movements, writing becomes small.
