Various tests are used in the diagnosis of Facial spasms. Some of these are listed below :
- Physical examination
- Examine the eye for local eye causes that may cause blepharospasm due to eye pain
- Examine for unilateral weakness of the facial muscles in hemifacial spasm
- Examine for Trousseau's sign which indicates hypocalcemia ( i.e. raise pressure of blood pressure cuff on arm above systolic pressure and after 2 minutes will see a contraction of the hand)
- Examine for Chvostek's sign which indicates hypocalcemia ( i.e. tap over the facial nerve under the ear and it will cause a brisk muscular twitch on the same side of the face)
- Test for increased reflexes which may occur in hypocalcemia

During a consultation, your doctor will use various techniques in their assessment of Facial spasms.
Your doctor may ask several questions when assessing
your condition. It is important to remember that your consultation is a two-way process and any extra information
you can share with your doctor about your symptoms may help them with their diagnosis.
Some of the questions your doctor may ask are listed below:
- How long have you had the facial spasms? - to determine if acute or chronic.
- Is it unilateral or bilateral?
- Is there a history of eye trauma? - may indicate a corneal abrasion or corneal ulcer may cause eyelid twitching.
- Past medical history? - e.g. hemifacial spasm may be related to acoustic neuroma, Paget's disease, Bell's palsy. Blepharospasm (spasm of the eyelids) may be associated with Parkinson's disease and progressive supranuclear palsy; Sydenham's chorea may follow within 3 months of rheumatic fever and recur in later life.
- Past surgical history? - e.g. previous parathyroidectomy or thyroidectomy may cause hypoparathyroidism.

- Eye pain? - Blepharospasm (involuntary contraction of the eye lids) may occur in response to any cause of eye pain. A painful eye may suggest herpes simplex infection, corneal ulcer, acute iritis, scleritis, episcleritis or acute glaucoma.
- Convulsions? - may cause facial spasms.
- Symptoms of hemifacial spasm? - e.g. irregular spasm of the facial muscles on one side of the face usually occurring in middle-aged women. It varies in severity from a mild inconvenience to a severe and disabling condition.
- Symptoms of hypocalcemia (low levels of calcium in the blood)? - e.g. numbness around the mouth, muscle cramps, anxiety, facial muscle twitching may be followed by convulsions, stridor and psychosis.
- Symptoms of Tourette's syndrome? - e.g. rapid repetitive and involuntary muscle movements and vocalizations called "tics', often involves behavioral difficulties, most often begins between the ages of 2 and 21.
