Diagnostic tests for Fingernail symptoms:

Various tests are used in the diagnosis of Fingernail symptoms. Some of these are listed below :

  • Physical examination
    • Inspect fingernails and toenails from the side to determine if there is true clubbing i.e. a loss of the angle between the nail bed and the finger. A subtle sign may be an increased in sponginess of the proximal nail bed
    • Inspect nail for nature of abnormality
    • Temperature, pulse rate, blood pressure
    • Heart and lung examination to detect any signs of cardiac or respiratory disease as a possible cause of finger clubbing

Diagnosis Checklist: Questions your doctor may ask (and why!)

During a consultation, your doctor will use various techniques in their assessment of Fingernail symptoms. Your doctor may ask several questions when assessing your condition. It is important to remember that your consultation is a two-way process and any extra information you can share with your doctor about your symptoms may help them with their diagnosis. Some of the questions your doctor may ask are listed below:

  • How long have you noticed problems with your fingernails?
  • How many fingernails are affected?
  • Are the toe nails affected also?
  • Risk factors for fungal nail infection? - e.g. wearing of occlusive footwear, overcrowding of toes, communal bathing areas, poor peripheral blood circulation.
  • Risk factors for iron deficiency anemia? - e.g. heavy periods, pregnancy, vegetarianism, peptic ulcer, gastrointestinal bleeding, celiac disease and frequent blood donations.

Diagnosis Checklist: Associated symptoms your doctor may ask about (and why!)

Sometimes, other symptoms may be present and may help your doctor analyse your condition. The symptoms your doctor may ask about (in association with Fingernail symptoms) may include:

  • Blue nails? - may suggest cyanosis, Wilson's disease, ochronosis, hemochromatosis or ingestion of certain medications.
  • Red nails? - may suggest polycythaemia and carbon monoxide poisoning.
  • Yellow nails? - suggests Yellow nail syndrome.
  • White nails? - may suggest hypoalbuminaemia (reduced albumin in blood) which may be due to chronic liver disease, severe malnutrition, nephrotic syndrome, protein losing gut disease.
  • Blueness of the tongue or lips? - If associated with finger clubbing determines presence of cyanosis and thus may suggest cyanotic congenital heart disease and pulmonary arteriovenous aneurysms. If associated with blue nails, may indicate hemoglobin abnormalities or cyanosis e.g. chronic obstructive pulmonary disease.
