Diagnostic tests for Hand tremor:

Various tests are used in the diagnosis of Hand tremor. Some of these are listed below :

  • Physical examination
    • Rapid pulse rate may indicate hyperthyroidism (especially if thyroid is enlarged) or caffeine sensitivity
    • Paroxysmal elevation of blood pressure (may suggest phaeochromocytoma)
    • Mini-mental state examination - if dementia is suspected test memory and cognitive function. If tremor is present with mental deterioration may suggest Wilson's disease or Parkinson's disease (late stages).
    • Full neurological examination. If hyperactive reflexes or up going plantar reflex (Babinski sign) this may suggest multiple sclerosis. If symmetrical tremor with nil neurological signs would suggest a familial senile tremor
    • Examine for signs of Parkinson's disease such as lack of facial expression, bent over posture, few spontaneous movements, shuffling gait, lack of normal arm swinging with walking, pill-rolling tremor (where movement of the knuckles are combined with movements of the thumb), increase tone of limbs, monotonous speech
    • Observe for chorea (non- repetitive abrupt involuntary jerky movements), Kayser-Fleischer rings (a green-brown pigment around the circumference of the cornea) due to copper deposition, signs of chronic liver failure that may suggest Wilson's disease

Diagnosis Checklist: Questions your doctor may ask (and why!)

During a consultation, your doctor will use various techniques in their assessment of Hand tremor. Your doctor may ask several questions when assessing your condition. It is important to remember that your consultation is a two-way process and any extra information you can share with your doctor about your symptoms may help them with their diagnosis. Some of the questions your doctor may ask are listed below:

  • How long have you had the hand tremor? - to determine if acute or chronic.
  • Is it unilateral or bilateral?
  • Is it intermittent or constant?
  • What makes the tremor occur or worse? - e.g. an Intention tremor occurs on hand movement and increases towards the target and suggests a familial (benign essential tremor) or senile tremor, cerebellar disease, multiple sclerosis, Wilson's disease, hereditary familial ataxia, alcoholism, alcohol withdrawal. A tremor occurring at rest would suggest Parkinson's disease or manganese poisoning. A fine tremor of the outstretched hands would suggest hyperthyroidism.
  • Age of the person with tremor? - if elderly and tremor is symmetrical and not associated with any other neurological symptoms or signs, this would usually suggest senile tremor.
