Misdiagnosis and Jaundice-like symptoms

Chronic liver disease often undiagnosed: One study reported that 50% of patients with a chronic liver disease remain undiagnosed by their primary physician. The reasons are multifactorial. Possible conditions include chronic hepatitis, cirrhosis of the liver, hemochromatosis, or other types of liver condition. Diagnostic tests for Jaundice-like symptoms:

Various tests are used in the diagnosis of Jaundice-like symptoms. Some of these are listed below :

  • Physical examination
    • Inspect color of the skin and sclera of the eyes - e.g. the yellow discoloration of the sclerae and the skin in jaundice results from elevated Bilirubin levels in the blood; skin pigmentation that is not bilirubin may suggest hemochromatosis as the cause of jaundice; examine for signs of anemia.
    • Abdominal examination for enlarged liver, enlarged spleen, ascites and palpable gallbladder which may be associated with various causes of jaundice and may help in determining the cause - e.g. if there is little or no enlargement o the liver this would suggest hemolytic anemia, Gilbert's disease , pernicious anemia and Dubin-Johnson syndrome; massively enlarged liver may suggest Gaucher's disease; enlarged gallbladder may suggest obstructive jaundice, cancer of the pancreas, cancer of the bile ducts or cancer of the ampulla of Vater; significantly enlarged spleen may suggest infectious mononucleosis, cirrhosis of the liver, hemolytic anemia, Gaucher's disease or Kala azar; edema and ascites would suggest alcoholic cirrhosis.
    • Look for evidence of chronic liver disease such as reddening of the palms, easy bruising, muscle wasting, testicular atrophy and enlargement of the breasts in males
  • Blood tests

During a consultation, your doctor will use various techniques in their assessment of Jaundice-like symptoms. Your doctor may ask several questions when assessing your condition. It is important to remember that your consultation is a two-way process and any extra information you can share with your doctor about your symptoms may help them with their diagnosis. Some of the questions your doctor may ask are listed below:

  • How long have you had the jaundice symptoms? - to determine if acute or chronic.
  • What jaundice symptoms do you have? - e.g. yellowing of skin, yellowing of the sclera of the eyes, pale stool, dark urine.
  • Is the person with jaundice a newborn baby? - Jaundice in a newborn is apparent clinically in 50% of term infants and more than 80% of premature infants. It is mostly physiological and benign. However, in a minority of babies it is a sign of serious underlying disease and should not be ignored e.g. blood group incompatibilities, infections, hypothyroidism, red cell membrane disorders, red cell enzyme deficiencies, Crigler-Najjar syndrome.
  • Have there been any previous episodes of jaundice?
  • Has there been any contact with people with jaundice?

  • Dark urine and pale stools? - occurs with obstructive or cholestatic type jaundice such as gallstones, cancer of the pancreas, cancer of the bile duct, strictures of the bile duct, some medications, recurrent jaundice of pregnancy.
  • Abdominal pain? - In a person with jaundice may suggest common duct stones, sclerosing cholangitis, pancreatic cancer, bile duct cancer, pancreatitis, viral or alcoholic hepatitis.
  • Itching of the skin? - suggests cholestatic liver disease such as viral hepatitis, alcoholic hepatitis, recurrent jaundice of pregnancy, primary biliary cirrhosis, common bile duct gallstones, cancer of the bile ducts, cholangitis, pancreatitis, biliary stricture, some medications.
  • Weight loss? - may suggest pancreas or bile duct cancer if associated with jaundice.
  • Fever? - In a person with jaundice may suggest cholangitis, viral hepatitis, pancreatitis or severe alcoholic hepatitis.
