Introduction: Lymph symptoms

The lymphatic system is part of the immune system and distributes immune cells and related fluid (called lymph) throughout the body. Small nodes called lymph nodes often swell in response to infection or disease (see swollen lymph nodes), which people sometimes misleadingly call "swollen glands". The lymph system can also directly be subject to illness itself, such as in lymphoma.

Diagnostic tests for Lymph symptoms:
Various tests are used in the diagnosis of Lymph symptoms. Some of these are listed below :
- Physical examination
- General inspection for pallor of the skin and conjunctiva may suggest leukemia, lymphoma or malignancy
- Examine all lymph nodes - including epitrochlear (in front of the elbow), Axillary, cervical (neck), occipital (back of head), supraclavicular (above the collar bone), inguinal (groin) and popliteal (behind the knee). Determine if lymph node enlargement is localized to one region (e.g. in local acute or chronic infection; early lymphoma especially Hodgkin's lymphoma or metastases) or generalized (e.g. in late lymphoma; leukemia; viral bacterial or protozoal infection; rheumatoid arthritis; systemic lupus erythematosus; sarcoidosis; phenytoin).
- Determine lymph node consistency. Hard nodes suggest cancer deposits. Soft nodes may be normal. Rubbery nodes may be due to lymphoma
- Determine if lymph nodes are tender which would suggest infection or acute inflammation

Diagnosis Checklist: Questions your doctor may ask (and why!)
During a consultation, your doctor will use various techniques in their assessment of Lymph symptoms. Your doctor may ask several questions when assessing your condition. It is important to remember that your consultation is a two-way process and any extra information you can share with your doctor about your symptoms may help them with their diagnosis. Some of the questions your doctor may ask are listed below:
- How long have you had the lymph symptoms? - to determine if acute or chronic.
- What lymph symptoms do you have? - e.g. swollen lymph nodes, tender lymph nodes or lymphedema (swelling especially in the subcutaneous tissues as a result of obstruction of the lymphatic vessels or lymph nodes and the accumulation of large amounts of lymph fluid in the affected area).
- If the lymph nodes are enlarged, are they focal or diffuse? - If enlarged lymph nodes are focal, should look for an infectious process in the area supplied by the respective lymph nodes.
- Medications? - Most medications may cause enlarged lymph nodes. The most notable is dilantin, but the antibiotics, aspirin, iodides and certain antihypertensive drugs can cause enlarged lymph nodes also.
- Sexual history? - may help in determining the risk of syphilis and HIV if generalized lymph nodes enlargement. If localized groin lymph node enlargement consider lymphogranuloma venereum (LGV), chancroid, granuloma inguinale and herpes simplex.

Sometimes, other symptoms may be present and may help your doctor analyse your condition. The symptoms your doctor may ask about (in association with Lymph symptoms) may include:
- Fever? - should consider infectious mononucleosis (glandular fever), brucellosis, dengue fever, toxoplasmosis and Still's disease. If fever is absent should consider Hodgkin's disease, chronic myeloid leukemia, polycythaemia, sarcoidosis, secondary syphilis, lymphosarcoma.
- Generalized itch? - must consider lymphoma.
- Non-pitting edema (skin is not indented when compressed) of an extremity? - would suggest lymphedema (swelling especially in the subcutaneous tissues as a result of obstruction of the lymphatic vessels or lymph nodes and the accumulation of large amounts of lymph fluid in the affected area).
- Genital ulcers or sores? - may suggest herpes simplex infection, primary or secondary syphilis, Donovanosis, Chancroid or Lymphogranuloma venereum which may all cause enlarged groin lymph nodes.
- Enlarged occipital lymph nodes (back of head)? - should consider dermatitis of the scalp, head lice, cellulitis, boil, fungal skin infection or a tick bite. However glandular fever or rubella may begin with enlargement of the occipital lymph nodes.
