Misdiagnosis and Neck swelling

Translucent swelling in Cystic hygroma: Cystic hygroma is a swelling usually occurring in the lower third of the neck most commonly in the posterior triangle, but may also occur in the axilla, groin and the mediastinum. It usually manifests in the neonate or in early infancy (occasionally present at birth). The swelling is soft and partially compressible and invariably increases in size when the child coughs or cries. The characteristic feature that distinguishes it from all other neck swelling is that it is brilliantly translucent. The other causes of neck swelling which can be confused with it are like soft tissue tumours, goitre, thyroglossal duct cyst, branchial cleft cyst. It may show spontaneous regression. Various tests are used in the diagnosis of Neck swelling. Some of these are listed below :

• Physical examination
  • Pulse rate. If elevated must consider Grave's disease, subacute thyroiditis or early Hashimoto's thyroiditis
  • Look for signs of hyperthyroidism including agitation, restlessness, enlarged thyroid (goiter), warm and sweaty palms, fine tremor, protruding eyes. May suggest Grave's disease, subacute thyroiditis or early Hashimoto's thyroiditis
  • Examine for signs of hypothyroidism - e.g. slow heart rate, slowed reflexes, coarse dry brittle hair, dry cool skin, goiter.
  • Watch neck lump when swallowing. If the lump moves with swallowing this would suggest thyroid lump or thyroglossal cyst

• Is the swelling intermittent or constant? - An intermittent swelling suggests pulsion diverticulum, venous distention of congestive cardiac failure, a bronchial cyst, a stone of Wharton's duct (stone in the submandibular salivary gland duct) and aneurysms.
• Past medical history? - e.g. cancer of the nasal cavity, pharynx, tongue, lips, mucous membrane (lining of the mouth), larynx and thyroid spread to neck lymph nodes.

May suggest Grave's disease, subacute thyroiditis (de Quervain's thyroiditis), toxic multinodular goitre or early Hashimoto's thyroiditis.
