Several severe illnesses lead to general pallor of the body ranging from anemia to leukemia to heart conditions. Localized pallor can also occur in areas of poor circulation such as the hands and fingers in Raynaud's phenomenon. Paleness of the skin needs to be distinguished from other symptoms such as whitening of the skin or hypopigmentation (loss of skin pigment). Any form of paleness or pallor needs medical investigation.

Diagnostic tests for Paleness:

Various tests are used in the diagnosis of Paleness. Some of these are listed below :

  • Physical examination
    • Look for signs of anemia - e.g. pallor of skin, pallor of nail beds and sclera of eyes.
    • Look for signs of heart failure including low blood pressure, increased pulse rate, heart murmur, crackles in lungs and swollen ankles
    • Looking for possible causes of anemia - e.g. signs of chronic liver failure, neurological signs suggesting possible pernicious anemia.
    • Examine for enlarged thyroid which may indicate presence of thyroid disease associated with vitiligo
    • Examine for nystagmus (jerky movements of the eyes), translucency of the iris and reduced visual acuity which are always present in albinism

Diagnosis Checklist: Questions your doctor may ask (and why!)

During a consultation, your doctor will use various techniques in their assessment of Paleness. Your doctor may ask several questions when assessing your condition. It is important to remember that your consultation is a two-way process and any extra information you can share with your doctor about your symptoms may help them with their diagnosis. Some of the questions your doctor may ask are listed below:

  • How long have you had the paleness of the skin? - to determine if acute or chronic. Albinism is present at birth. Some causes of sudden acute paleness include fainting, shock, motion sickness, heart attack, heart rhythm problem, hypoglycemia (low blood sugar attack), severe gastroenteritis, leukemia or bleeding disorder.
  • Is the paleness localized or generalized? - Generalised paleness of the skin suggests albinism, phenylketonuria, fainting, shock, motion sickness, heart attack, heart rhythm problems, hypoglycemia (low blood sugar attack), severe gastroenteritis, leukemia or bleeding disorder. Localised paleness suggests Raynaud's phenomenon, vitiligo, post-inflammatory hypopigmentation, pityriasis versicolor or exposure to certain substances that are toxic to pigment producing cells.
  • Is there a simple explanation for the paleness? - e.g. stress, physical exertion, lack of sun exposure, normal genetic variation, motion sickness.
  • Presence of precipitating factors for vitiligo? - e.g. physical trauma, illness, emotional stress, following the death of a relative, following sunburn.
  • Predisposing factors for pityriasis versicolor? - e.g. high humidity, Cushing's syndrome, prolonged administration of both topical or oral corticosteroids.
