Diagnostic tests for Paralysis symptoms:

Various tests are used in the diagnosis of Paralysis symptoms. Some of these are listed below :

  • Physical examination
    • Blood pressure - if blood pressure is high must consider a stroke (due to a cerebral hemorrhage, cerebral thrombosis or cerebral aneurysm).
    • Pulse rate and rhythm looking for atrial fibrillation which would suggest a cerebral embolism as a cause of a stroke
    • Cardiovascular examination looking especially for murmur and an enlarged heart
    • Listen for a carotid artery bruit which can indicate narrowing of the arteries to brain and thus risk factor of stroke

Diagnosis Checklist: Questions your doctor may ask (and why!)

During a consultation, your doctor will use various techniques in their assessment of Paralysis symptoms. Your doctor may ask several questions when assessing your condition. It is important to remember that your consultation is a two-way process and any extra information you can share with your doctor about your symptoms may help them with their diagnosis. Some of the questions your doctor may ask are listed below:

  • How long have you had the paralysis (loss of power of voluntary movement in a muscle)? - to determine if acute or chronic.
  • Which muscles does the paralysis affect? - e.g. hemiplegia is complete loss of power of the limbs of one side and is usually, but not always due to a lesion within the brain such as a stroke or tumor; paraplegia is complete loss of power of both lower limbs and is usually due to a spinal cord lesion; facial paralysis is usually due to a lesion of the seventh cranial nerve.
  • Is there total paralysis of the muscles or weakness of the muscles? - The terms hemiplegia and paraplegia strictly indicate total paralysis. Hemiparesis means weakness of the limbs of one side. Paraparesis means weakness of both lower limbs.
  • Is the paralysis intermittent? - may suggest migraine, transient ischemic attacks from the basilar artery or carotid artery disease, epilepsy.
  • Was the onset sudden or gradual? - Sudden onset of hemiplegia would most commonly suggest a stroke (due to cerebral thrombosis, hemorrhage or embolism); however a contusion (bruising) of the spinal cord may also produce a sudden onset of paralysis. A history of trauma and a sudden onset of hemiplegia (complete loss of power of the limbs of one side) would suggest a subdural or epidural haematoma (blood clot pressing on the outside of the brain). A gradual onset of hemiplegia may suggest a brain tumor or brain abscess. A sudden onset of facial paralysis would suggest Bell's palsy, diabetic neuropathy and stroke as cause. If gradual onset of facial paralysis must consider acoustic neuroma, advancing petrositis, brain tumor or brain abscess.

Diagnosis Checklist: Associated symptoms your doctor may ask about (and why!)

Sometimes, other symptoms may be present and may help your doctor analyse your condition. The symptoms your doctor may ask about (in association with Paralysis symptoms) may include:

  • Limb pain? - a painful joint or muscle disease may interfere with assessment of leg power.
  • Paralysis of the facial muscles? - If facial paralysis is associated with complete loss of power of the limbs of one side of the body, should suspect a cause within the brain. If paralysis of the facial muscles is absent, more likely to suggest a cause in the spinal cord.
  • Fever? - If associated with hemiplegia may suggest cerebral abscess, venous sinus thrombosis or encephalitis. If associated with paraplegia may suggest venous sinus thrombosis, abscess of the spinal cord or myelitis. If associated with facial paralysis must consider mastoiditis or middle ear infection.
  • Seizures? - may suggest brain cancer or metastasis, encephalitis, stroke.
  • Headaches? - may suggest brain cancer or brain metastasis, subdural or extradural haematoma, stroke.
