Diagnostic tests for Proteinuria:

Various tests are used in the diagnosis of Proteinuria. Some of these are listed below :

  • Physical examination
    • Blood pressure - e.g. if high blood pressure may suggest hypertension induced proteinuria, preeclampsia, glomerulonephritis, polycystic kidneys, lupus erythematosus, heart disease.
    • Inspect for edema (swelling) of ankles, legs, face, abdomen and genitals
    • Examine for the butterfly shaped facial rash of systemic lupus erythematosus
    • Abdominal palpation for mass in flanks - e.g. bilateral flank massesmay suggest polycystic kidneys; a unilateral flank mass may suggest a hypernephroma (kidney cancer).

Diagnosis Checklist: Questions your doctor may ask (and why!)

During a consultation, your doctor will use various techniques in their assessment of Proteinuria. Your doctor may ask several questions when assessing your condition. It is important to remember that your consultation is a two-way process and any extra information you can share with your doctor about your symptoms may help them with their diagnosis. Some of the questions your doctor may ask are listed below:

  • When was the proteinuria (excessive protein excreted in urine) discovered?
  • Is the patient a child and has suffered a Streptococcal infection - ( e.g. tonsillitis, pharyngitis, middle ear infection or cellulitis) 1-3 weeks before the onset of the proteinuria? - suggests post streptococcal glomerulonephritis. Hemolytic uremic syndrome is a disorder of infancy and childhood that follows a febrile illness, particularly gastroenteritis or an upper respiratory tract infection.
  • Are you pregnant? - may suggest pregnancy induced hypertension, pre-eclampsia or eclampsia. Any person with systemic lupus erythematosus who gets pregnant is at risk of rapidly progressive glomerulonephritis.
  • Have you had an injury such as a blow to the loin? - Trauma to the kidneys may cause proteinuria.
  • Past medical history? - e.g. diabetes, amyloidosis, systemic lupus erythematosus, systemic sclerosis, multiple myeloma, congestive cardiac failure, high blood pressure.

Diagnosis Checklist: Associated symptoms your doctor may ask about (and why!)

Sometimes, other symptoms may be present and may help your doctor analyse your condition. The symptoms your doctor may ask about (in association with Proteinuria) may include:

  • Fever? - Temperature may suggest urinary tract infection or lupus erythematosus. A fever itself may also cause proteinuria.
  • Butterfly shaped facial rash? - suggests systemic lupus erythematosus.

  • Symptoms of nephrotic syndrome? - e.g. swelling of the ankles and legs, swelling of the abdomen, face and arms may also be present (especially in children). Swelling of the genitals is sometimes seen.
