Infestation, especially
   Hookworm
   Onchocerciasis
   Ascariasis
   Trichinosis
Drugs
   Opiates
   Aspirin
   Estrogens, progestins, androgens
   Erythromycin
   Sulfonylureas
   Phenothiazines
   Quinidine
   Vitamin B complex
   Psoralen/ultraviolet A radiation (PUVA)
Obstructive liver disease
   Bile duct obstruction, extrahepatic
   Bile duct obstruction, intrahepatic
       Biliary cirrhosis
       Drug-induced cholestasis (oral contraceptives, phenothiazines, chlorpropamide)
       Intrahepatic cholestasis of pregnancy
Neoplasm, especially
   Lymphoma, leukemia
   Abdominal malignancy
   Central nervous system malignancy
   Paraproteinemia
   Carcinoid tumor
Polycythemia vera
Iron deficiency
Diabetes mellitus
Hyperthyroidism
