The list of diagnostic tests mentioned in various sources as used in the diagnosis of Bloody semen includes:

  • Physical examination
    • Temperature
    • Abdominal examination for enlarged and tender bladder due to urinary retention as complication of benign prostatic hypertrophy, prostate cancer or prostatitis
    • Digital rectal examination e.g. prostatitis has exquisitely tender, swollen, firm and warm prostate; benign prostatic hypertrophy of the prostate has an enlarged prostate; prostate cancer may have a hard, nodular and irregular gland
    • Urine or ejaculate microscopy and culture - In prostatitis often has white cells and bacteria but may be normal. Urinary tract infection may complicate benign prostatic hypertrophy.
    • First pass urine PCR for Chlamydia and gonorrhea - if suspected as cause of prostatitis.
    • Post-massage prostatic secretions - may show excess white cells with prostatitis.
  • Blood tests
    • Renal function tests which may be affected with urinary obstruction due to prostatitis , benign prostatic hypertrophy or prostate cancer
    • Prostate specific antigen (PSA) levels may be elevated in prostate cancer, prostatitis and benign prostatic hypertrophy. Normal levels do not exclude these conditions
  • Radiological investigations
    • X-Ray of prostate - may show prostatic stones that may prevent successful treatment of prostatitis.
    • Transrectal ultrasound scan
  • Prostatic needle biopsy - (if indicated) if suspect prostate can.
