          Viral (especially entero-, arbo-, adenovirus; HIV infection)
          Rickettsial (Rocky Mountain spotted fever, louse-borne typhus)
          Miliary tuberculosis
          Malaria
       Anaphylaxis
       Neoplasm (especially leukemia, lymphoma, and carcinoma of lung, prostate, or pancreas)
       Thrombotic thrombocytopenic purpura
