PENILE SORES

Ask the Following Questions:

  1. Is it painful? The presence of a painful penile sore suggests chancroid, herpes simplex, herpes zoster, and balanitis. On the other hand, a painless penile sore should suggest chancre, lymphogranuloma venereum, epithelioma, granuloma inguinale, and papilloma.
  2. Is there inguinal adenopathy? If there is inguinal adenopathy, lymphogranuloma venereum, epithelioma, and chancre should be suspected.

DIAGNOSTIC WORKUP

Routine studies include a smear and culture of material from the sore and a dark field examination. A VDRL test should be performed if syphilis is suspected. Many cases require a biopsy of the lesion or a biopsy of the lymph node.

A Frei test will help diagnose lymphogranuloma venereum. A Tzanck test, serologic test, and viral isolation will help diagnose herpes zoster and herpes simplex. There are also serologic tests for lymphogranuloma venereum, if necessary. A Lygranum test may help diagnose lymphogranuloma inguinale. Difficult diagnostic problems should be referred to a urologist.
