The list of diagnostic tests mentioned in various sources as used in the diagnosis of Red tongue includes:

  • Blood tests
    • Full blood count and ESR
    • Iron studies
    • Folate
    • Vitamin B12
    • Parietal cell antibodies (present in 90% of people with pernicious anemia)
    • Intrinsic factor antibodies (present in 50% of people with pernicious anemia)
    • Schillings test, if suspect pernicious anemia
    • Erythrocyte glutathione reductase activity ( Vitamin B2 status) - not widely available.
    • Erythrocyte aminotransferase activity (Vitamin B6 status) - not widely available.
    • ANA autoantibodies, if suspect systemic lupus erythematosus
  • Biopsy of tongue may be indicated in some situations such as solitary tongue ulceration that does not respond to treatment
