Diagnostic Tests for Sexual pain

Diagnostic Test list for Sexual pain:

The list of diagnostic tests mentioned in various sources as used in the diagnosis of Sexual pain includes:

  • Physical examination
    • Examine the genital area and note any lesions - e.g. ulcer, lump, redness, sores or swellings.
    • Examine any genital discharge, if present
    • Vaginal speculum examination to observe any discharge and lesions within the vagina and to allow swabs to be taken
    • Manual vaginal examination to examine for any masses, tenderness or enlarged uterus. If a mass around the ovaries or fallopian tubes is felt may suggest salpingo-oophoritis (infection of the ovaries and fallopian tubes), endometriosis with a cyst, malignancy ovarian tumor or ectopic pregnancy
    • Look for evidence Peyronie's disease (non-tender hard patch may be felt just beneath the skin of the penis)
    • If the penis is erect, determine if the head of the penis is erect (hard) also. If the head of the penis is not hard but the rest of the penis is erect, this suggests priapism
    • Rectal examination to detect hemorrhoids, anal fissures and severe constipation which may cause sexual pain. Digital rectal examination to assess prostate size, consistency and tenderness
    • Feel the lymph nodes in the groin for enlargement which may suggest penile cancer, herpes simplex or Chancroid
  • Blood tests
    • Full blood count and ESR
    • Pregnancy test, if suspected
    • Blood cultures, if fever is present
    • Tumor markers such as CA 125, HCG and alpha fetoprotein may assist in diagnosis and management of malignant ovarian tumor
    • Prostate specific antigen (PSA) levels
  • Urine tests
    • Urinalysis to detect cystitis or bladder stone
    • Urine microscopy and culture
    • Urine pcr to detect Chlamydia and gonorrhea
  • Vaginal swab for microscopy and culture
  • Swab of cervical canal and rectum for microscopy and culture to test for gonorrhea and Chlamydia may also be necessary
  • Swab and culture the base of any genital lesions - to diagnose genital herpes, syphilitic ulcer, Chancroid (Haemphilus ducreyi).
  • Urethral discharge sample - for microscopy, culture and PCR looking for organisms that may cause urethritis including Chlamydia, gonorrhea, trichomoniasis, ureaplasma urealyticum, mycoplasma genitalium and Candida.
  • Radiological investigations
    • Pelvic ultrasound, if there is a pelvic mass found on physical examination or if suspect ectopic pregnancy
    • Transrectal ultrasound, if suspect prostate cancer
  • Pap smear
  • Biopsy of the vulval skin, if suspect vulval dystrophy, dermatitis, psoriasis, lichen simplex chronicus or vulval cancer
  • Skin biopsy may be required to diagnose penile cancer
  • Laparoscopy may be needed
  • Exploratory laparotomy may be the only way to make a diagnosis in the case of a pelvic mass
