Stretch marks are common in obese patients or during pregnancy, in which case they are of no pathologic significance. However, purple striae of the abdomen, especially when they are associated with moon facies or a buffalo hump, should immediately call to mind Cushing's syndrome. Patients on prolonged corticosteroid therapy also develop purple striae. The workup is the same as for Cushing's syndrome.
